High School diploma
At least 1 years of experience. Working with individuals with mental illness and/ or intellectual disabilities or a combination of related experience and education, preferably community based.
Applicant must have a reliable vehicle and be willing to utilize it on a regular basis
Duties Include:
Assist in the development of an individual Housing Support Program service assessment and goal plans for each consumer, review them with the consumer on a regular basis and update them as needed.
Assist and/or instruct consumers in activities of daily living (budgeting, banking, grocery shopping, meal preparation, medication organization, laundry, housekeeping and general maintenance.)
Assist consumers with any housing related issues in their homes with landlords, family members and members of the community.
Provide instruction and support to consumers learning to utilize community resources including transportation.
Assist consumers in developing natural social and community supports.
Assist consumers in scheduling, attending and participating in recreational activities as requested by the consumer and in developing hobbies and interests for personal leisure time.
Develop and lead social, support and educational groups as determined by the needs and wishes of consumers.
Assist consumers in accessing appointments as indicated by the consumers' service plans.
